Try forcing fsck on boot with something like - `shutdown -r -F now`.
Check for file corruption using rpm -V, especially if your hard drive is not on a standard ide controller, specifically HighPoint. --I am pretty sure there is a bug in the HighPoint driver causing file corruption but I can't nail it down, might just be my hardware.

Might simply be makewhatis barfing on UTF8 problem but check the file system.
