Please show "mdadm -E /dev/sdo1" I'm guessing that drive letters have changed since you first put this together. Could you please get my "lsdrv" script from: http://github.com/pturmel/lsdrv and show us the output. This will help make sure we don't lose track of the roles of each of these drives. My first impression from the above output is that the shrink from 13 back to 12 did not actually happen. Shrinking is dangerous enough that mdadm stays in test mode unless you set a sysfs variable with the new size before trying to shrink. Another question: Do you have a backup?
