No, you don't need to patch the kernel after upgrading mdadm.
Some things might not work in older kernels that do work in newer kernels,
but if you have a reason to stay with 2.6.18, then do so.

That patch is only needed if you want to use the --incremental functionality
of mdadm.
If you do want to use that functionality, then you should really
upgrade your kernel.
If you don't then don't worry about the patches.

I'll probably remove them from the next release of mdadm.
