Why no 'default device' option?

What PA version is that? Also, IIRC fallback only works for new streams,
so if you have something playing and connect the USB headset, it won't
move on-the-fly (in PA >= 0.9.20 it should work for manually moved
streams). But if you start playing after the headset is connected, all
streams that haven't been moved around manually should be played there.

You can try removing your ~/.pulse to start with a clean slate. Log
out / in (or kill PA) to make sure it starts afresh, and select the USB
headset as fallback. See in pavucontrol if it will be selected
automagically every time you connect it (it should...).

As long as module-default-device-restore is loaded, it should handle
that. And all new streams should play in the new fallback device.
