res_pjsip overrides this functionality to allow the Max-Forwards value 
to be configurable. If it's not working then I'd suggest filing an 
issue[1] with the complete configuration files and the version of 
pjproject in use. If you'd like to investigate further on the Asterisk 
side code level wise it's set as a global request header in 
res/res_pjsip/config_global.c which uses 
res/res_pjsip/pjsip_global_headers.c
