PUBLISH arbitrary presence body with pjsua-lib

Hi Klaus

Maybe there is a better way but indeeed when I did this I implemented a new

pjsua_acc_set_online_status

It is a bit messy since I added some new feature to the rpid implementation so these elements are added at the same time as other xml elements are built. I think there are probably better ways to do this. Are there not ways to extend the presence packages in pjsip, I think I saw that once.
