You should format the email message content first, like this:
$msgContent = "Name: ". $values['name'] ."\n";
$msgContent .= "Address: ". $values['address'] ."\n";

Then you should send a this content, like this:
mail('email@xxxxxxxxxxx', 'Subject', $msgContent, "From...);
