Try:
require("../initiate.php");
One dot doesn't do anything (that I know of).
Also, place the following in your code (initiate.php), run it, and
find the correct path:

echo("<br/>");
echo("<pre>");
print_r($_SERVER);
echo("</pre>");
?>
