The cause, I surmised, was that someone was stopping the download of the page before it finished, so the code execution stopped, or they were double clicking the submit button.
The cause was a bit hazy, though ignore_user_abort() fixed it for me. Is it possible, in your case, that the code is partially executing, and giving you the impression it's sending dublicate mails?
