try using Neatimage (free) - it can do noise reduction and sharpening, and if used carefully can really 'tidy' up skin blemishes, even to the point of making people look waxy and plastic (if you go too far!)

it has a standalone or if you want you can get the photoshop plugin - I recommend the standalone.
