After ruling out camera motion, subject motion and examining the original scanned file which shows some grain on extreme enlargement I have come to the conclusion that the UV filter on the lens was dirty enough to degrade the image sharpness. Kodak showed this degradation effect of a dirty lens verses a clean lens in a darkroom book many years ago.

Jack up the contrast.  It will make the "dirt" less visible.  Same with fog.
