Sample from Linux-PAM docs doesn't work for me

I compiled the PAM example program from the Linux-PAM documentation,
but I can't get it to work.

1. It always wants to authenticate the user that's running the app, as
   opposed to the username that's given in pam_start.

2. I can't get it to authenticate other than with this pam config:

auth required pam_unix.so
account required pam_permit.so

What am I doing wrong?
