I might also suggest replacing 'Professional Role' with something like 'Relevant Expertise', which might be information about a relevant profession, but might be about other non-professional expertise. More generally, this is a lot of information- my eyes glazed over about half-way through. Unless you've got a particular plan to utilize all this information in a way which requires it to be structured, I'd recommend asking for no more than a biography paragraph- 'this is who I am; this is why I have relevant skills'.
