I was mounted the device after making a FS on it using the command
mke2fs.
I could not understand your statement "first time device opens and
second time onward it fails to open". Could you please explain the steps
that are you doing?

As I said, I didn't mount the device. Could you tell me exact steps you
followed to make fs and mount?
