You have failed to include any useful information that would allow us to help you. Most likely is that you are using a low performance or buggy audio output driver. Less likely but still remotely possible is a slow performing machine or video output driver. However to do any diagnosis you will have to provide much more info. Have you at least done any research by googling some common terms like "mplayer audio out of sync mkv" ??? That is where I would start.
