[merged] lib-extablec-add-missing-prototypes.patch removed from -mm tree

The patch titled
     Subject: lib/extable.c: add missing prototypes
has been removed from the -mm tree.  Its filename was
     lib-extablec-add-missing-prototypes.patch

This patch was dropped because it was merged into mainline or a subsystem tree

When building with W=1, a number of warnings are issued:

Add the missing #include for the prototypes.
