[merged] dma-mapping-use-offset_in_page-macro.patch removed from -mm tree

The patch titled
     Subject: dma-mapping: use offset_in_page macro
has been removed from the -mm tree.  Its filename was
     dma-mapping-use-offset_in_page-macro.patch

This patch was dropped because it was merged into mainline or a subsystem tree
