New user setup

I've installed the latest release and executed successfully against a
126 Mb unix mail file containing messages posted since 1994. I got 1 big
maillist.html (and 1 threads.html) file as output.

At a minimum, how do I get the index files split up by month, i.e.:
http://www.mhonarc.org/archive/html/mhonarc-users/

Ideally, how do I get the 'pretty' html interface for each month, i.e.:
http://www.mhonarc.org/archive/html/mhonarc-users/2004-07/index.html
