The Page Layout section lists the resources that control the layout and
appearance of generated HTML pages.  For each type of page generated,
there are resources that control the start and end of each page.  You
can redefine those resources (via a resource file) to define the
headers and footers you want.

The HEADER and FOOTER resources only apply to the main index page.
The documentation gives you the alternate resource to use to achieve
the same desired effect.
