The Carnegie Mellon library provides a CAPTCHA that is, apparently,
effective, in that it's hard for computers and easy for people. The
library is in the process of digitising their collection, and
when their software has trouble recognising some text, they make it into
an image for a CAPTCHA. It's a little more complicated than that, but
the result is that the responses they get are used for digitising the
library.

The monotone developers are currently adapting it to ikiwiki.
