[xhci] corrupt urbs after module reload

Hallo Sarah,

looks like my adventure with this controller are not finished.
Short story:
After reloading ath9k_htc module i get some corrupt urbs on adapter side. It was possible to confirm it by debuging ath9k_htc firmware. On the host side i tried to use Wireshark, but at this stage all affected urbs was ok.
After reloading xhci_hcd module, my adapter work ok.

Right now i was able to reproduce this issue only on:

00:14.0 USB controller: Intel Corporation 7 Series/C210 Series Chipset Family USB xHCI Host Controller (rev 04) (prog-if 30 [XHCI])
	Subsystem: ASUSTeK Computer Inc. Zenbook Prime UX31A
	Flags: bus master, medium devsel, latency 0, IRQ 41
	Memory at f7d00000 (64-bit, non-prefetchable) [size=64K]
	Capabilities: <access denied>
	Kernel driver in use: xhci_hcd


same adapter with same firmware has no issue with other XHCI controllers, for example:

03:00.0 USB controller: Fresco Logic FL1009 USB 3.0 Host Controller (rev 02) (prog-if 30 [XHCI])
	Subsystem: ASUSTeK Computer Inc. Device 1427
	Flags: bus master, fast devsel, latency 0, IRQ 19
	Memory at de000000 (64-bit, non-prefetchable) [size=64K]
	Memory at de011000 (64-bit, non-prefetchable) [size=4K]
	Memory at de010000 (64-bit, non-prefetchable) [size=4K]
	Capabilities: <access denied>
	Kernel driver in use: xhci_hcd

or other EHCI controllers.
