type 2 is "internal" and reason 204 (0xcc) is "unknown cause code".

Not very helpful, but FWIW something I've seen from time to time.  I
don't really know, but this might be something the modem and/or network
doesn't like about the session you are attempting to start.  The
currently available QMI tools are not very good at this.  But you might
be able to set up the necessary using AT+CGDCONT.
