[Pull request] xHCI patches for 2.6.40?

Hi Greg,

Here's the finalized version of the patchset for Intel Panther Point
xHCI support.  I would appreciate it if you can do a late pull request
for Linus for this, as it really is needed for 2.6.40.  If you can't,
let me know and I'll break it up into two pull requests for bug fixes
for 2.6.40 and features for 2.6.41.
