[PATCH 0/5] EHCI: isochronous scheduler changes

Greg:

The following patches include a bunch of changes to the isochronous 
scheduler in ehci-hcd plus related code:

	1/5 fixes a bug that could affect anybody using a non-PCI EHCI 
	controller.

	2/5 introduces a micro-optimization (replace remainder with 
	bitwise-and).

	3/5 fixes a bug (failure to convert from frames to 
	microframes).  However it is liable to uncover problems in
	other drivers, so it is not recommended for -stable.

	4/5 reorganizes iso_stream_schedule(), improving the code
	structure and allowing longer transfers to be accepted.

	5/5 removes dead code.
