Changelog from v1: - Remove redundant define #if 0 for irq handler. - Split xhci_request_irq for setup_msi and setup_msix and move the code directly into xhci_setup_msi/xhci_setup_msix - Free irq vector before calling pci_disable_msix. - Refine some goto statement. - Cleanup xhci_free_irq. - Remove unnecessary comments. - Modify un-nest if loop in xhci_setup_msix.
