Yes, it seems likely that there is a failure of the hardware or the 
firmware.  Have you tried plugging the phone into different computers?

The trace shows what you described: The phone worked until you switched 
it into storage mode.  Then it disconnected, reconnected, and did not 
work again.

Maybe it will work better if you put the phone into storage mode, then 
turn it off and on again.
