The usbmon log shows what the problem is.  Something in your system is 
trying to pass data in high memory to dummy-hcd, which can't handle it.

It's probably a bug somewhere in the block layer.  You should try using
a more recent kernel, such as 2.6.30 or 2.6.31-rc4.
