This would be easier to follow without all the extra debugging 
information clogging up the log.

That's the problem right there.  Notice that there's a two-minute delay 
after the device is plugged in before the error occurs.  What happened 
during that delay?

More information is needed.  Please capture a usbmon trace and post the 
result (see Documentation/usb/usbmon.txt in the kernel source for 
instructions).
