My apologies, this was in fact a bug in our code: we did not define the address-level ACL correctly.

I would try to see what is the actual error, e.g you can use tcpdump and look on the iscsi login response, and/or run iscsid -f -d 127 & and see the prints it outputs, and/or run tgtd -f -d 1 & and see what error it prints, etc.
