I can't reproduce it on demand but it commonly seems to happen when quite a bit of text is scrolling on the screen (eg. when booting and boot messages are scrolling on the screen). Although I used the example of boot messages I have encountered it at other times like when I was browsing in lynx and it was loading a new page.
