Did you use
rpm -ihv kernel*.rpm
where kernel*.rpm is the speakup-modified one?
That should work.
I am not running fc4 so have not tried it.
Once installed, you might need to edit grub.conf to make sure that the 
speakup-modified kernel is the one that will boot.
