[GIT PULL] SCSI fixes for 3.12-rc6

This is a set of 2 fixes which cause oopses (Buslogic, qla2xxx), 1 fix
which may cause a hang because of request miscounting (sd) and four
updates to the eas2r driver which was a new driver in the last merge
window and is thus eligible to have updates under the new driver rule.
