[GIT PULL] hopefully final SCSI fixes for 2.6.35-rc6

This just represents the five remaining bug fixes I know about, one in
ipr, one in the IBM vscsi driver and three in zfcp (so nothing x86
impacting).  The fix resource formatting actually fixes a buffer
overflow which causes an oops.

The patch is available from
