PROBLEM: 3ware 9000 controller module panic with 2.6.12.5 kernel

Hello,
I am having a constant problem with getting my 3ware-9xxx controller working stable under 2.6.12.5 kernel on Fedora Core 3, Intel Pentium 4 3.2Ghz with Hyperthreading disabled.
The motherboard is SUPER MICRO P4SCI, there are 15 SATA drives from Western Digital.

I am using the latest 3ware driver from amcc.com (9.2.1 release), 2.6.12.5 kernel compiled with kdb debugger patches.
In attachments I also include some information from kdb (those are screenshots in GIF format) and various TXT attachments which include information from /proc/ioports, /proc/iomem, /proc/scsci/scsi, lspci -vvv output).
