[PATCH] pci/vmd: Move vmd driver to drivers/pci/host

This moves the driver source and Kconfig to the pci host bridge drivers
directory, relocating the config option to a more approrpiate sub-menu
instead of occupying the top level location.

The Kconfig option for VMD has been updated with its X86_64 dependency
that was implicitly included from the previous location, and added
missing information for building this driver as a loadable module.
