Try to find icons on your server:
$ find / -name anthony-dirup.gif -print

Look into your /etc/squid/squid.conf configuration file for this
directive:
icon_directory /usr/lib/squid/icons

#  TAG: icon_directory
#       Where the icons are stored. These are normally kept in
#       /usr/lib/squid/icons

Are your icons in thos directory?
