With poor network connectivity you want uucp connections not toys like
fetchmail. UUCP will get you basically 100% of what you can out of any dialup 
link including compression and minimising phone time. Something the IP folks
dont do.

Modern UUCP setups support non bangpath routing or bsmtp (batched-smtp) which
gives you all the internet mail system behaviour without the overhead you pay
for when you use IP

Finally having UUCP transfers available lets you use some of the very old DOS
setups that will do email on even an old 8086 and while not open source are
free to grab pieces.
