As already printed, on Ubuntu are some back ported media modules in unusual places. You need to be root to get them removed or have to do it manually to avoid duplicate modules. At least the digital tuner is not found at 0x60 and the card can't work. Have you forced other cards previously, since it also should be auto detected? Wrong tuner initialization code can make i2c unreliable. You might try to unload the driver starting with saa7134-alsa and saa7134-dvb, eject the card then and wait at least 30 seconds before you give it another try. You could also try to enable i2c_debug=1 for saa7134. Maybe more errors become visible, dunno.
