People normally apply patches as they come so what happens is that
no one replies and the v1 patch is applied and the v2 set is thrown
away.

When Greg applies a patch, he sends me a message to inform
me but for the others maintainers you are right I don't know
whether they already applied it or not. I will be more careful.
No I'm not saying that. I'm saying that people don't even see the
v2 patch until it's too late because no one responded to the v1
thread. The v2 is buried hundreds of emails later in their inbox.
