Another bit of information I have noticed and is possibly helpful, the issue
only seems to present itself after a suspend/resume cycle.  After that, it
usually occurs within 5 minutes of resuming the laptop.  Once it does, and I
force the BT device to reset with the rfkill trick in comment #1, it seems
fairly solid after many hours of use.
