Want to know what was stopping it working?  Do you?

USB hub.  Yep, a nasty little bit of $10 hardware.

The keyboard was connected through a USB hub.  When I traced through the 
wiring, found this and plugged directly into the computer, it all 
started working.

Any USB gurus like to tell us why that was?
