No USB hub, connected directly. The error from the kernel is:

cannot submit datapipe for urb 4, err = -28

And nothing else. I have usb bandwidth turned off in the kernel. ALSA and OSS emu drivers will playback sound. It is only a JACK issue.

which parameters are you using to start JACK?

48kHz, and periods/buffer = 3  seem to be preferred by USB audio devices.

There should be something about this in the archive of this list too..
