usb-midi is the OSS module, you have to load the snd-usb-audio module,
which is part of ALSA.

Jack is an audio server, AFAIK for now it knows nothing about MIDI (even if
qjackctl has a facility MIDI-tab under "setup"). I know that MIDI trough
jack is planned, but I guess it will use ALSA as driver.

Try the snd-usb-audio module, and put these lines into
/etc/hotplug/blacklist:

usb-midi
snd-usb-audio
