The  Driver=snd-intel8x0m occassionally dies when loaded, or between
successive dialouts.
So you have to periodically:
$ sudo modprobe -r  snd-intel8x0m
$ sudo modprobe snd-intel8x0m
thus unloading and reloading the driver.

If you are already on PPP when the modem dies and using the preferred
wvdial dialout utility.
First stop wvdial with:
Ctrl-C
before unloading and reloading the driver
