Ankur,

If you have installed the sl-modem-daemon package,
uninstall it because it is useless for the snd-hda-intel case and may
interfere. Uninstall with:
$ sudo apt-get remove sl-modem-daemon

Then run
$  find /usr  -name  slmodemd
which will locate the slmodemd .  This is usually at /usr/sbin/slmodemd
If
$ slmodemd --version
reports version 2.9.11, all is OK.  if not rerun the slmodemd install
from the SLMODEMD.gcc4.3.tar.gz
Afterwards again check
$ slmodemd --version
