scanModem reports that your modem is supported in principle by SLMODEMD.

The audio card hosts a softmodem chip:  0x10573057

The softmodem chip 0x10573057 is in principle supported by the COMM
support of slmodemd

Download:

http://linmodems.technion.ac.il/packages/smartlink/SLMODEMD_gcc4.3_alsa1.0.17.tar.gz

Unpack
under Linux with:
     $ tar zxf SLMODEMD_gcc4.3_alsa1.0.17.tar.gz
 and read instructions therein.

Briefly, the modem is setup with command:
     sudo slmodemd -c USA  --alsa hw:0,6
 reporting dynamic creation of ports:
     /dev/ttySL0 --> /dev/pts/N   , with N some number
 Read DOCs/Smartlink.txt and Modem/DOCs/YourSystem.txt for follow through
guidance.

It did not generate the instructions for
SLMODEMD.gcc4.3-alsa-1.0.17.tar.gz correctly?

Here you should have used:
$  sudo slmodemd -c USA  --alsa hw:0,6
because scanModem should have reported this, could have been a bug?

Instead of the command you used you should have tried:

$  sudo slmodemd -c USA  --alsa hw:0,6
then proceeded with
$ sudo wvdialconf /etc/wvdial.conf

and hopefully found the modem port /dev/ttySL0  .
