Hogat if you got it to work in /etc/modprobe.conf, then the line /bin/mknod -m 660 /dev/slamr0 c 242 0 2>/dev/null && chgrp uucp /dev/slamr0 is not needed in /etc/rc.d/init.d/slmodemd script. Try removing that line. Also please remove the service momemntarily.
