 The drivers are in the kernel modules tree at:
 /lib/modules/2.6.24-19-generic/ubuntu/sound/alsa-driver/pci/hda/snd-hda-intel.ko
 The modem codec file for the HDA card is: /proc/asound/card0/codec#1

11c11040 is a LSI/Agere modem chip hosted on the  High Definition Audio Card.

get the  agrsm-ubuntu8.04.1-2.6.24-19-generic.tar.gz
Under Linux unpack with:
$ tar zxf agrsm*.tar.gz
Move into the new folder and browse a while
$ cd agrsm-ubuntu8.04.1-2.6.24-19-generic
Then install the drivers with:
$ sudo ./setup

As further help is needed report to the List.
I'm not the expert for this modem.
