Download from http://linmodems.technion.ac.il/packages/smartlink/ the slmodem-2.9.11-20080629.tar.gz, which has a fax/ subfolder with some guidance which may be useful. The slamr driver mentioned therein is one of the alternate drivers working under slmodemd control.
