Download from http://linmodems.technion.ac.il/packages/smartlink/
the slmodem-2.9.11-20080629.tar.gz,  which has a fax/ subfolder with
some guidance which may be useful.  The slamr driver mentioned therein
is one of the alternate drivers working under slmodemd control.

Aside from that we lack fax expertise on this list,
and you should consult with the efax list
