scanModem reports:

Intel 537

The output is not clear.  There appears to be two
modems and a sharing issue:

Something does not look right, maybe slmodemd does not
work for this modem and it needs conexant/linuxant
drivers.  I'm forwarding this to list so that they
recommend something for you.
